Link employees to community resources & doctors
Close to 20% of the employees we have screened do not have a current family doctor or have not been to a doctor in the last five years.
- Health screenings can provide awareness for employees and decrease health care costs, both for individuals and for corporations.
For employees without primary care physicians, ICH will work with your health plan to assist employees to obtain a doctor within the network.
Participants can opt-in to have their screening results forwarded to their primary doctor and/or specialist.
An "a la carte" menu of options is available for follow-up
ICH offers other follow-up programs including an incentive walking and basic nutrition program, newsletter, and diabetes and cardiovascular disease management program.
Incentive tracking program
Helps participants initiate and sustain healthy lifestyle change behaviors.
Serves as a tracking tool for participants to log the following:
- Number of minutes of exercise
- Daily consumption of fruits and vegetables
- Bi-monthly education and recipes
Tracking tool calculates point totals based on a participant’s entered values.
- Participant aims for milestones at 10, 20 and 30 weeks and achieves prizes when each benchmark is met.
Encourages camaraderie.
- Company can choose to implement point challenges between departments, locations or even "buddy teams" comprised of two individuals
Diabetes and Cardiac management tools
- Nurses, Certified Diabetes Educators, dietitians, and other healthcare personnel administer programs.
- Educational sessions include books and electronic information and are distributed to employees.
- Participants receive reminders for medication refills, doctor’s appointments, and annual tests.
Educational programming, including nutrition and weight management
- Programs may be delivered on-site or remotely.
- If our staff cannot provide the program, we work with local vendors to ensure high quality of service.
